Pellet Therapy

There are two major pellet therapy types: estrogen pellets and testosterone pellets. These pellets are inserted into the upper buttocks area during an in-office visit. Before the insertion, extensive testing is done to determine the exact quantity of the hormone the patient requires. Each dose is customized based on a detailed consultation and comprehensive lab work to ensure that the patient receives the precise concentration of hormones needed.

Hormones are evaluated through a blood test panel, with results reviewed individually. To address hormonal imbalances, we employ subcutaneous pellets, requiring a minor office procedure.


During the in-office visit, which only takes a few minutes, patients are escorted to the exam room where the insertion site is numbed. Typically, the upper buttocks is the chosen site. A small incision is made to insert the pellet, and after insertion, the site is covered with a small bandage.

After pellet insertion, it’s advisable to refrain from lower body exercise. Walking is acceptable, but avoid strenuous activities. Steer clear of bathtubs, swimming pools, and hot tubs, although showers are permitted. Once the bandage is removed, you may resume normal activities.

Since pellets are bioidentical and contain hormones similar to those our bodies naturally produce, they are fully dissolved and absorbed within the body, leaving no residue behind.

Most patients experience no pain during hormone pellet insertion. To ensure comfort, a local anesthetic is applied to the insertion site beforehand. Some tenderness, redness, and minor bruising may occur for up to 3-4 days post-insertion, but the entire process is generally quick and painless.

Pellets typically last between three to six months, depending on factors such as gender, weight, absorption rates, and the existing levels of hormones at the time of insertion. To maintain optimal and balanced hormone levels, pellets are reinserted two to four times per year. It may take up to two insertions before patients experience the full benefits.

Each person’s body is unique, but as a pellet cycle concludes or when discontinuing pellet therapy entirely, symptoms such as fatigue, low mood, and sleeplessness may resurface. Typically, the patient’s symptoms will revert to their state prior to BHRT as their estrogen and testosterone levels decline.
